Inspector Notes
An unannounced monitoring inspection was conducted 4/3/2023 from 11:05am through 12:00pm. There were 38 children directly supervised by 6 staff. The physical plant, programming, menu, 5 staff records, 5 children?s records, 5 medications, emergency drills, emergency supplies, and policies were inspected. Children were observed playing a freeze dance game and eating lunch. Positive interactions between children and staff were observed during this inspection. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations
3Evidence: Child #3 and Child #5 were both present at the time of inspection and did not have documentation of immunizations on site.
Evidence: Two emergency medications were on site for Child #8 on 4/3/2023. There was no written authorization from the parent on site.
Evidence: Child #7 has a diagnosed food allergy and didn't have a written allergy care plan on file signed by a physician.
