Inspector Notes

An unannounced monitoring inspection was conducted beginning at 7:15 a.m. and ending at 8:15 a.m. with the director. There were 13 children in care ages 6 through 14 years old under the supervision of 4 staff. Children were observed participating in zones.

Four children's records and four staff records were reviewed. Two medications and allergy care plans were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff files, the center failed to obtain a repeat criminal history background check for all staff every five years as required. Evidence: Staff #1 most recent national fingerprint background check was dated 5/1/2018 and a new background check should have been obtained by 5/1/2023.
