Inspector Notes

An unannounced renewal inspection was conducted with the director on August 28, 2023 beginning at 3:28pm and ending at 4:16pm. There were 27 children, ages 6 and 12 years old, in the care of 2 direct care staff. The children were observed arriving to the program and attendance being taken. The children were observed during free time, playing games, reading, and on their school computers. Five children?s records were reviewed and in compliance. Two staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. There were no medications on site to be reviewed. The most recent emergency escape drill was documented as 6/7/2023. The most recent shelter-in-place was documented as 4/6/2023. The most recent health inspection was dated 2/28/2023. The most recent fire inspection was dated 02/08/2022.

Based on observation and staff interview, the center did not have the appropriate staff-to-child
ratio for children up to 9 years old. Evidence: On August 28, 2023 from 3:40pm to approximately 3:45pm there was one staff member supervising 27 children ages 6 through 12 years old.
Plan of Correction: The center will ensure that there are two staff members at the center prior to the children being dismissed from school. There were two staff scheduled to be at the center but one was not able to be there due to illness.
