(Repeat Violation)The Center is required to maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that is accessible to all staff in each group or area where children are present.

The allergy list did not contain two of Child #1`s diagnosed food allergies and one of Child #2?s diagnosed food allergies.

(Repeat Violation) The Center is required to have written medication authorization from the child?s physician and parent for a long term medication.

For a medication for Child #2, there was no parent or physician authorization on file.
For a medication for Child #5, there was no parent or physician authorization on file.
For a medication for Child #6, there was no parent authorization on file.
Plan of Correction: Correct forms in file.

(Repeat Violation) The Center is required to obtain documentation for child records that includes the name, home address, home phone number, work phone number and place of employment of each parent who has custody.

The file for Child #3 and Child #4 was missing the name, address, phone number, work place and work number for a custodial parent.

(Repeat Violation) The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

1)During inspection, there were 37 children present, but the written record reflected there were 10 children present.
2)Upon arrival to the Center, children were moving in and out of the classroom without permission and staff were not aware of the number of children in the classroom in their care or the number of children in the halls and bathroom areas in their care.
Plan of Correction: Paper attendance is taken
before the iPad is filled out.
Staff has a new form for
attendance that is being
checked daily to insure they
are following it and
completing it.
Snack time has been changed
and students will not be
washing hands until after
everyone has been checked in.
