Inspector Notes

An unannounced renewal inspection was conducted with the director on October 11, 2023, beginning at 4:35pm and ending at 5:22pm. There were 39 school-aged children in the direct care of seven staff members. The children were observed playing with toys and games during the inspection. Six children?s records were reviewed. Three staff records were reviewed and in compliance. Six medications were reviewed and in compliance. Three allergy care plans were reviewed. Facility policies and procedures were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 9/29/23. The most recent shelter-in-place drill was documented as 9/21/23. The most recent lockdown drill was documented as 9/21/23 The most recent health inspection was dated 3/8/23. The most recent fire inspection was dated 1/19/23.

Based on a review of six children?s records, the facility failed to ensure that all the children?s records contained the required information. Evidence: Child A?s record was missing a written care plan for a child with a diagnosed food allergy.
Plan of Correction: The facility staff will be requesting the missing documentation from the parents.
