Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 03/28/2025. The on-site inspection began at 11:45 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 24 children present and 5 staff. The inspector reviewed six children?s records and five staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. A tuberculosis (TB) screening is required every two years from the last screening.

The repeat TB screening for Staff A was due two months ago and had not yet been completed.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

Child A has emergency medication as well as an allergy care plan for a food allergy that was not listed on the center's allergy list.
