During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/29/24. The on-site inspection began at 4:35 pm and ended at 5:30 pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 4 staff. The inspector reviewed 7 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Immunization reports shall be signed by a physician or their designee.
The immunization records for child #2 and child #3 were not signed by a physician or their designee at the time of enrollment. Both children have been enrolled for over 2 months.
Plan of Correction: I will obtained signed copies of the immunization records.
Standard 8VAC20-780-510-H
Medications that are dispensed from a pharmacy must be maintained in the original, labeled container.
Prescription medication for child #1 was not maintained in the original, labeled container from the pharmacy.
Plan of Correction: I will obtain the original box or prescription label.
