During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/22/2025. The on-site inspection began at 7:00am and ended at 7:50am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/31/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician: Before the child's attendance or within 30 days after the first day of attendance.

Child A and Child B, both enrolled for 2 months, did not have documentation of physical examinations in their files.
Plan of Correction: Documentation has been printed and placed in file.
Standard 8VAC20-780-150-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.

Child A and Child B, enrolled for 2 months, did not have immunizations records that were signed by physician, his designee or an official of a local health department.
Plan of Correction: Kaiser does not sign forms- had mom take to see if they could be stamped.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present.

Staff could not provide a written list of all children?s allergies, sensitivities and dietary restrictions.
