Inspector Notes
A monitoring inspection was initiated on 8/31/2022 and concluded on 8/31/2022. There were a total of 8 children ( 21 points) observed with 3 caregivers present. The children were engaged in the following: toy play, blocks, and dinosaurs. The home, 2 child records, 7 caregiver/household member records, evacuation drills, emergency supplies and policies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
2Evidence: Household #1 does not have record of fingerprint results. Provider stated that she is residing in home but provider has not obtained her fingerprints yet.
Evidence: Household #2, date of birth 7/31/2008 does not have results of central registry check once turning 14 years of age. Provider stated that she has not completed the form for the central registry results for the household member.
