Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 03/10/2023, between the hours of 10 am to 11:30 am. There were 6 children present (16 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 6 children records, 1 household record, and 1 staff record was reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-60-B
Based on review, 1 of 6 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (03/10/2023), at least 2 emergency contacts were not listed for Child #6 (start date - 11/09/2022).
2. Child #6 had the information listed for one emergency contact.
Plan of Correction: This information will be obtained and placed on file.
Standard 8VAC20-800-90-C
Based on review, 3 of 6 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (03/10/2023), the following children had immunization documentation dated older than 6 months: Child #1 (dated - 06/24/2022); Child #3 (dated - 07/15/2022); and Child #5 (dated - 08/02/2022).
2. All three children listed above were under that age of 2 years on the date of inspection.
Plan of Correction: Updated immunization pages will be obtained for all applicable children.
