Inspector Notes

A Subsidy Health and Safety Inspection was conducted on (date) to review supplemental health and safety requirements. There were 4 children present, ranging in ages from 3 to 4 years old, with 1 staff supervising. Both inspections combined cover subsidy health and safety regulations.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all the subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

No violations of the supplement subsidy health and safety requirements were found.
