Inspector Notes

An unannounced complaint inspection was conducted on 06/06/2023 from 10:05 a.m. to 10:45 a.m. The complaint was received on 5/27/2023 regarding staffing and supervision, special care provisions and emergencies.

The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?not valid.?. One non-complaint related violation was documented on the violation notice issued to the program.

Based on interview, the provider failed to inform the department?s representative as soon as practicable, but no more than two business days, of any injury to a child while under the vendor?s supervision when a referral is made for treatment from a medical professional.
Evidence:
1. On 05/30/2023, the Department received a complaint regarding an injury that occurred on 05/25/2023.
2.Interview with the vendor on 06/06/2023 confirmed that on 05/25/2023, Child #1 received medical treatment due to a knee injury that occurred from jumping off a piece of play equipment while Child #1 was under the supervision of the vendor.
3. The vendor confirmed that she failed to report the injury to the department?s representative.
