Inspector Notes

An unannounced, on- site monitoring inspection was initiated on July 17, 2025and completed on July 17, 2025. The on-site inspection began at 10:10 a.m. and ended at 11:10 a.m. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was four. The inspector reviewed one child record and three caregiver/household members records on-site. inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 28, 2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Household Member #3 was not completed and was overdue by nine months.

Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies shall be stored away from food in areas inaccessible to children.
In the bathroom, accessible to children, was Lysol, 2 cans of air freshener, rubbing alcohol, disinfecting wipes and toiler bowl cleaner.
Plan of Correction: Immediately removed all substance.
