Inspector Notes

An unannounced renewal visit and facility tour was conducted on 02/29/2024. At the time of inspectors' arrival there were 18 infant to preschool aged children in care with 4 staff members. Children were observed interacting with staff, nap, snack and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that a central registry check was completed on each staff member within 30 days of employment.
Evidence:
1. There was no central registry search results in the record for staff #1(hired 2/7/23).
2. Staff reported they have had trouble getting this search completed.
Plan of Correction: Staff will contact OBI to follow up on this request or resubmit search request.
Standard 8VAC20-780-130-E
Based on record review it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child #2, under 2 years of age, did not contain documentation of updated immunizations. The documentation available was dated 2/7/23.
2. The record for child #3, under 2 years of age, did not contain documentation of updated immunizations. The documentation available was dated 8/25/23.
Plan of Correction: Staff will request documentation from parents.
