Inspector Notes

A monitoring inspection was initiated and concluded on 04/11/2024. The inspector arrived at 2:45 PM and departed at 3:30 PM. There were 34 school aged children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 site staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview. the program director provided the plans of correction for the violations cited.

Based upon review of records, the facility has not ensured that there is an annual inspection conducted by the fire marshal.
Evidence:
The most recent documentation of a fire marshal inspection is dated 8/30/2022.
Plan of Correction: The facility responded with the following:
The fire marshal has already been contacted requesting an inspection, however the inspection has not been conducted. Follow up with the fire marshal will be taken.

Based upon review of injury records, the facility has not ensured that the written record of children's injuries includes all required documentation.
Evidence:
Two injury reports dated 3/8/2024 for child 1 did not includes the dates and times the parents were notified of child 1'2 injuries.

In the future the program director will ensure that all required information is documented. All staff will be reminded of injury reporting procedures and documentation.
