During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 1/10/2025. The inspection began at 3:10 PM and ended at 4:14 PM. The inspector reviewed compliance in the areas listed above. There were 17 children present and 4 staff. The inspector reviewed 6 children?s records and 6 staff records. This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on February 20, 2025. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The results of the most recent VDOE inspection were not posted.

Potable water testing: The plan established pursuant to subsection A and the results of each test conducted pursuant to such plan shall be submitted to and reviewed by the Commissioner and the Department of Health's Office of Drinking Water.

The center shall maintain a written record of children's serious and minor injuries that include all required documentation.
An injury record form reviewed does not have the date and time the parent was notified of the child's injury. Another record has only one signature.
Plan of Correction: The facility responded with the following:
Youth Director will audit and maintain a written record of children's injuries. Staff training will be conducted to ensure staff understand and carry out this standard.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information.
The record provided for child 4 did not include the name, address and telephone number of the father.
