During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 11/14/24 and completed on 11/15/24. The on-site inspection began at 4:10pm and ended at 5:15pm. The inspector reviewed compliance in the areas listed above. There were six children present and two staff. The inspector reviewed four children?s records and three staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/25/24. A POC submitted after this date will not appear on the public website.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

There was no allergy list at the program on 11/14/24.
Plan of Correction: The Allergy list had been removed due to the site being relocated on 11/13/24. The Allergy list has been reposted.
Standard 8VAC20-780-60-A
The center shall maintain and keep at the center a separate record for each child enrolled that contains all required information.

The records for Child C and Child D, enrolled for more than three months, were missing the addresses for two designated persons to contact if parents cannot be reached in an emergency.
Plan of Correction: The address's were written on the registration forms for Child C and Child D.
