Inspector Notes
An unannounced inspection was initiated on 4/1/2025 and completed on (date) in response to a complaint received by the licensing office on 3/25/2025 relating to allegations of improper ratio. The inspector reviewed five children?s records and three staff records electronically on 4/3/2025. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable code sections. No violations were issued.
