Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 08/11/2021 and concluded on 08/19/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 17 children, ages 2 years-7 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, physical plant, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed during the inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to the first day of employment.

Evidence: The record for Staff #1 contained documentation that Staff #1 has resided outside of the state of Virginia with the preceding 5 years. The record for Staff #1 had a documented hire date of 06/10/2021; the sex offender registry check was dated 08/12/2021 and was not obtained prior to the first day of employment.
Plan of Correction: Sex offender registry checks will be done prior to hiring.
