Inspector Notes

An unannounced monitoring inspection was conducted on 05/09/2024 in response to a self-reported incident. The inspector reviewed photographs, audio recording of interviews conducted by outside agencies, interviewed 5 staff members, and reviewed a total of 3 children records and 2 staff records during the investigation. The inspector arrived at 1:00 pm and departed at 2:00 pm.

Information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
