Inspector Notes

An unannounced renewal inspection was conducted on 03/05/2024 from 9:40am to 12:20pm. There were 33 children ages 2 years old to age 8 years old supervised by 4 staff. The physical plant, programming, attendance, 6 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were reviewed. There were no medications onsite. Children were observed participating in art, table toys, singing, dancing, and drawing. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on staff interviews and observation, the center did not ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Evidence:
During inspection in the Preschool classroom (age 2), a child was observed leaving the classroom to go to the bathroom down the hall and out of sight and sound supervision of a staff member.
Plan of Correction: This was discussed with the staff the same minute when it happened it just happened quick and she will continue doing all the headcounts and because the gate was getting replaced so a new gate was replaced the same day so no one has access going to that.

Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence:
1)During inspection in the Preschool classroom, there were 8 children present, but the written record reflected there were 33 children present.
2)During inspection in the Toddler classroom, there were 4 children present, but there was no written record present in this classroom.
Plan of Correction: Toddler classroom were marked in the same main sheet the one we've been using all those years and on the school app on our devices. We will make sure they have their own attendance sheets at all time and not to put it all in one sheet. We did new attendance sheets same day.
