Inspector Notes

An unannounced on-site inspection was conducted on 1/10/2024. There were 61 children, ages 2 months-5 years, and 11 staff members present during the inspection. The inspector observed the following: free choice activities, circle time, and lunch. A total of 5 children?s records and 6 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 9:10 AM and departed at 12:15 PM.

Standard 8VAC20-780-160-A
Based on interview and review of staff records, the center failed to ensure that all staff had a negative tuberculosis (TB) screening at the time of employment, no more than 30 days prior to hire, and before coming in contact with children.

Evidence:

1.Staff # 1 had a documented employment date of 08/01/2023 and the TB screen was not completed no more than 30 days prior to hire and before coming into contact with children. The TB screening was dated 08/03/2023.
2.Staff # 5 had a documented employment date of 08/14/2023 and the TB screen was not completed no more than 30 days prior to hire and before coming into contact with children. and the TB screen was dated 01/30/2022.
Plan of Correction: In the future all TB screenings and Test will be completed per the standard.
Standard 8VAC20-780-340-D
Based on observation, staff record review, and interviews, the center failed to ensure that each grouping of children had a qualified program lead.

Evidence:

1.Staff #1 was observed working in the preschool room as a program leader on the day of the inspection. Staff # 1 stated he was the program lead. After review of Staff # 1 record, there was no documentation of the educational requirements for a program leader position.
Plan of Correction: Center is working on obtaining and providing training for program leader.
Standard 8VAC20-780-40-M
Based on observation of the posted list of
allergies, the center failed to ensure that the list is dated.

Evidence:

The allergy lists posted in all the classrooms were not dated.
Plan of Correction: All allergy postings will be dated every time it is updated.
Standard 8VAC20-780-80-A
Based on review of the attendance and interviews with staff, the center failed to ensure that for each grouping of children, the center maintained a written record of daily attendance that documented the arrival and departure of each child in care as it occurred.

Evidence:

1.On the day of the inspection at approximately 9:20 AM, the toddler class did not have the daily attendance completed for the six children present in that classroom. Staff # 6 confirmed the child # 2 was marked present on the attendance, child # 2 arrived at 10:40 AM.
Plan of Correction: The center will ensure that each grouping of children will have a completed attendance that documents the arrival and departure of each child as it occurs.
