During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/13/2025. The on-site inspection began at 9:00 a.m. and ended at 11:50 p.m. The inspector reviewed compliance in the areas listed above. There were 63 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-320-B
Each restroom area provided for children shall have sinks located near the toilets and that are supplied with running warm water that does not exceed 120?F.

The water in PreK Castle room measured 128?F.
Plan of Correction: Provider will have the water temperature lowered.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The fall zone on the small playground measured to be as little as 11 inches when each piece of equipment required at least six feet of a fall zone.
Plan of Correction: Provider will create a fall zone around equipment.
