During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 07/14/2025. The on-site inspection began at 9:30 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 56 children present and 10 staff. The inspector reviewed 5 children?s records, 5 staff records, and 2 agent/applicants on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff 1 TB was dated three months prior to date of employment and Staff 4 TB was dated two months prior to date of employment.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1.In the ?kitchen? area of the Treasure Cove room there was a table with chipped and splintered wood on the top. Children were observed using this table during play.
2.The landscaping fabric on the big playground was exposed in several areas creating a tripping hazard.
Plan of Correction: The table was removed during the inspection and the landscaping fabric will be removed.

The required ratio for children Birth to 16 months is one staff per 4 children.

There was one staff member and one volunteer in the Safari room with 7 children (ages 2 months to 12 months) for approximately 1 hour and 45 minutes. Volunteers do not count in the ratio.
Plan of Correction: Provider will ensure staff are in place to meet the required ratio or ensure all volunteers have the required documents.
