Inspector Notes

A complaint was received and initiated on May 12, 2022. An onsite investigation was completed on July 26, 2022. The investigation included in person and telephone interviews, record review a tour of the center and classroom observation. The complaint alleges inappropriate discipline. The information gathered does not support the allegation and the complaint is not valid. Violations found but not reported include staff tuberculosis screening and physical plant. These violations were discussed with the owner at the conclusion of the inspection.

Standard 8VAC20-780-160-A
Based on record review the center failed to ensure that tuberculosis documentation has been completed within the last 30 calendar days of the date of employment.
Evidence:
1 - The record of staff 2 contains tuberculosis documentation that is dated February 11, 2022.
a. Staff 2 has a documented date of employment as July 11, 2022.
b. Staff 2 was present providing direct care to children during the inspection.
2 - Staff 1 confirmed that the tuberculosis documentation on file for staff 2 was dated more than 30 prior to her date of hire.
Plan of Correction: Going forward center management will ensure that tuberculosis screenings are obtained within allotted time frames.
Standard 8VAC20-780-270-A
Based on observation the center failed to ensure areas and equipment are maintained is safe condition.
Evidence:
There were areas of peeling paint within reach of children on the climb and slide on the center playground.
Plan of Correction: Center management will ensure that areas of peeling paint are scraped and repainted as soon as possible.
Standard 8VAC20-780-330-B
Based on observation the center failed to maintain resilient surface as required.
Evidence:
1 - There was less than six inches of resilient surface under, around into the fall/use zone of the climb and slide playground equipment.
2 - There was grass and weeds growing up through the resilient under, around into the fall/use zone of the climb and slide playground equipment.
Plan of Correction: Center management will ensure that proper amount of resilient surface is obtained for the playground equipment.
