Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 08/30/2021 and concluded on 09/01/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 19 children present, ranging in ages from 7 months to 11 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies, nutrition and background checks. A total of 2 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record reviews, the center failed to obtain out of state background checks prior to staff's first day of employment.
Evidence:
1. The inspector reviewed four staff records. One of four records reviewed (staff record #4) documented that staff had lived in another state outside of Virginia in the last five years. Staff record #4 did not contain an out of state sex offender registry check or out of state criminal history name check. Staff #4's first date of employment was 08/02/2021.
Plan of Correction: The director completed the required document to request the out of state sex offender registry check and the out of state criminal history name check for the staff. The director will send the results of all of the out of state background checks for staff #4 to the licensing office once they are obtained. In the future, out of state background checks will be requested and obtained as required.
