Violations

1
Standard 8VAC20-770-40-D-5
Based on a review of records on February 21, 2024, the provider failed to ensure that each household member obtained a central registry check within 30 days of turning 14 years of age.

Evidence:
1. The record of household member #1 (DOB 09/06/08) does not contain a central registry result.
3. The provider acknowledged she did not obtain central registry checks for household member #1.
Plan of Correction: I will be sure to get the background check completed immediately in the portal.
