Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 04/03/2025, as part of the licensure period.

The on-site inspection began at 11:00 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above.

There were 9 children present and 2 caregivers; the point total was 28. The inspector reviewed 3 children?s records and 3 caregiver records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 04/10/2025. A POC submitted after this date will not appear on the public website.

Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver #2 completed 8.5 hours and Caregiver #3 completed 12 hours of training in the year 2024.
Plan of Correction: Caregiver will take trainings on time.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The record for Child #2, who was in care today and has been in care for six months, did not contain documentation of immunizations.
Plan of Correction: Parent's brought infant's immunization record at pickup time.
