An unannounced monitoring inspection was initiated on February 14, 2025 in response to a self-report received by the licensing office on January 31, 2025 relating to an injury. The inspector reviewed 1 child?s record and 1staff records on-site on February 14, 2025. This inspection included document review, interviews, and observations (recording).

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 26, 2025. A POC submitted after this date will not appear on the public website.

The following actions or threats thereof are forbidden: rough handling a child. On January 31, 2025 at approximately 3:44 p.m., Child 1 (2 years old) sustained an injury while in care of Staff 1 in the twos' classroom. Video shows that Staff 1 pulled the child up by his left arm to a standing position. Child 1 immediately began to cry and express that his left wrist was in pain. Staff 1 told Staff 2 that she "grabbed his hand and pulled him up". Child 1 was seen by a physician, and was diagnosed with nursemaid's elbow in his left arm.
Plan of Correction: All staff were given training and reminders of proper protocol when picking up children.
