Inspector Notes

An unannounced monitoring inspection was conducted today between 1:00pm and 2:15pm. There were 10 children present with the Provider and 2 assistant for a total of 20 points within the supervision guidelines. The children were observed doing yoga and transitioning to naptime. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-100-A
Based on record review, each child's record did not contain documentation of a physical exam.
Evidence: Child #4's record did not contain a physical exam.
Plan of Correction: I had it but the parent took it to make a copy. I will ask them to return the physical.
Standard 8VAC20-800-180-A
Based on interview, the provider did not obtain a subsequent tuberculosis (TB) screening every two years for each employee.
Evidence: The most recent documented TB screening for Caregiver #3 is dated 4/16/2021. Caregiver #3 confirmed that they had not yet received a repeat TB screening.
Plan of Correction: I will speak with [them] about making an appointment.

Based on record review, children's records did not contain all required information.
Evidence: Child #5's record does not contain the families medical insurance policy number.
Plan of Correction: I will have the family fill it out.
