During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on 10/9/2025 in response to a complaint received by the licensing office on 9/30/2025 relating to allegations of caregiver qualifications. In addition to the complaint investigation, a monitoring inspection was initiated and completed. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 16. The inspector reviewed 4 children?s records and 3 caregiver/household member records. This inspection included document review, a tour of the family day home, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the complaint, violations were found related to the monitoring inspection and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/27/2025. A POC submitted after this date will not appear on the public website.

The provider confirmed that they were out of the country during the months of July and August. There was no documentation of the arrival and departure times that the substitute provider was working on behalf of the provider.
Caregiver #2s most recent TB screening is dated 101/2023. The TB screening has been expired for approximately one week.
There were four uncovered outlets observed accessible to children under the age of five years.
Child #3 record does not contain documentation of the families medical insurance policy number.
The most recent documented fire drill is dated May 2025. The provider confirmed that fire drills were not practiced June, July, August and September.
