Inspector Notes

A renewal inspection was initiated and concluded on April 20th, 2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children's records, 1 caregiver record and 4 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Based on a review of household member records, the provider failed to obtain a sworn statement or a criminal history record check within 30 days of a person becoming 18 years old.

Evidence:
Household member #4 does not have documentation of a sworn statement or a criminal record check for a person in the home turning 18 years old.
Plan of Correction: Household member #3 signed a Sworn Statement today, April 20th, 2021
I will obtain a criminal record check for HHM #4 - sending form off today.
Standard 63.2-1720.1-A
Based on a review of caregiver and/or household member records, the center failed to obtain repeat background checks every five years.

Evidence:
Caregiver #1 - sworn statement was dated 8/14/2015 and central registry dated 10/06/2015.
Household member #1 - sworn statement was dated12/07/2015 and central registry was dated 10/06/15.
HHM #2 - sworn statement was dated 12/17/2015 and central registry was dated 10/06/2015.
HHM #3 - central registry was dated 10/06/2015.
