Inspector Notes

An unannounced monitoring inspection was conducted on December 13, 2022. There were 4 children (12 points) with 1 caregiver supervising. The home, children's records, caregiver records, household member records, attendance logs, evacuation logs, and evacuation logs were reviewed. Standard 8VAC20-800-100-A
Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1, #2, #3 and #4's records do not contain documentation of a physical.
Plan of Correction: I will get the information and put in the child's file.
Standard 8VAC20-800-60-A
Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
- Child #1, #2, #3 and #4's records did not contain documentation of the phone number for each custodial parent's place of employment.
- Child #3's record did not have documentation of one emergency contact or the names of persons other than parents allowed to pick up the child.
- Child #1's record did not contain documentation of the review of the child's emergency contact information as required by 8VAC20-800-780-B.
Plan of Correction: I will get the information from parents and put in the child's file.
