Inspector Notes

An unannounced monitoring inspection was conducted on May 3, 2023. There were 5 children present (16 points) with 1 caregiver. The home, children's records, caregiver records, household member records, first aid kits, menu's, carbon monoxide detectors, refrigerator thermometers and cribs were reviewed. The children were observed hand washing and playing with floor toys. Areas of non-compliance were cited on the violation notice. Standard 8VAC20-800-100-A
Base on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child # 3, #4 and #5's records do not contain documentation of a physical examination by the 30th day of attendance.
Plan of Correction: I will ask the parents for the physicals and put them in the child's files.
Standard 8VAC20-800-180-A
Based on a review of household member records, the provider failed to obtain for each adult household member a current Report of Tuberculosis Screening form every two years for the date of the first screening.

Evidence:
Household member #2's record does not contain documentation of a current Report of Tuberculosis Screening form.
Plan of Correction: I will have household member complete a TB screening and will put information in file.
Standard 8VAC20-800-60-B
Based on a review of children's records, the provider failed to obtain all required documents for each child enrolled.

Evidence:
-Child #1, #2, #3, #4 and #5's records do not contain documentation of the name and phone number of each custodial parent's place of employment.
-Child #5's record does not contain documentation of the addresses of two designated persons to contact in case of an emergency, if the parent cannot be reached.
-Child #1, #2, #3, and #4's records do not contain documentation of names of persons, other than the custodial parents, authorized to pick up the child.
Plan of Correction: I will get the information from the parents and place in children's files.

Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized before the child may attend the family day home.

Evidence:
Child #1, #2, #3 and #5's records do not contain documentation of immunizations by the first day of attendance.
Plan of Correction: I will ask the parents for the immunizations and will place then children's files.
