Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/1/2025. The on-site inspection began at 11:25pm and ended at 12:25pm The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 10. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site on 5/1/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider is required to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Child #1 did not have documentation of a physical examination on file at the time of the inspection and has been enrolled for over two years.
Plan of Correction: I will ask the parents for it.
