Inspector Notes
An unannounced renewal inspection was conducted today between the hours of 11:15am-1:30pm. There were 7 children directly supervised by the provider and 2 additional caregivers. The home, 3 caregiver records, 3 children?s records, 2 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed finishing lunch, playing freely, and participating in storytime. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

6Evidence:
1. The record for Caregiver #1 did not contain the following:
-Verification of age;
-Date of employment or volunteering;
-Name address and telephone number of a person to be notified in an
emergency;
Evidence:
1. Provider stated Caregiver #1 began employment in February 2023. The documentation of tuberculosis screen/testing in the record for Caregiver #1 was dated 12/21/2022.
2. Caregiver #2 began employment on 1/23/2023. The documentation of tuberculosis screen/testing in the record for Caregiver #2 was dated 6/16/2022.
Evidence:
1. The most current tuberculosis screening in the record for Caregiver #3 was dated 09/28/2020.

requirements in 8VAC20-800-170, every two years from the date of the first
screening or more frequently as recommended by a physician or the local health
department.
Evidence:
The record for Child #1 did not contain the following:
- policy number of the child's medical insurance
-address for one of the two designated persons to contact in case of an emergency if the parent cannot be reached.
Evidence:
The most recent review of the emergency plan documented was dated 3/3/2021.
Evidence:
1.The evacuation drill log for the Family Day Home did not contain documentation of evacuation drills practiced for the months of February 2023 and March 2023.
