During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/22/2024, as a part of the licensure period. The on-site inspection began at 10:40am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 6 children present and three caregivers; the point total was 15. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-170-B
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The form shall have been completed within the last 30 days.
The tuberculosis screening in the record for Caregiver #3, who had been caring for children for three weeks, was completed 8 months prior to their first day.
Plan of Correction: The caregiver will obtain a current tuberculosis screening.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. An emergency evacuation drill was not completed for the month of April 2024.
Plan of Correction: Emergency drills will be practiced monthly.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. The last documented immunization report in the record for Child #1, age 23 months, was over 8 months ago.
Plan of Correction: The provider will request the documentation from the parents.
