During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 03/24/2025, as a part of the licensure period. The onsite inspection began at 10:10am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 20. The inspector reviewed 5 children?s records and 6 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-100-A
Providers are required to obtain documentation of a physical examination within 30 days after the first day of attendance. The records for Child #2 and #3 did not contain documentation of a physical examination. The physicals for Child #2 and #3 are one month overdue.
Plan of Correction: The provider has reminded parents that the physicals are due. Parents have provided the forms to the doctor to be completed.
Standard 8VAC20-800-40-A
The provider shall ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. The maximum licensed capacity of the home is 7 and there were 8 children in care during the inspection.
Plan of Correction: Provider reported one child would not be returning to the home until another child discharges from care in two weeks.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. The record for child #4 did not have a parent?s signature for an annual review of the emergency contact information. The annual review was overdue by one month.
Plan of Correction: Provider will ask the parents to review the information today.
Standard 8VAC20-800-830-A
Providers are required to practice shelter-in-place procedures a minimum of twice per year. There were no shelter in place drills documented for the year 2024. Caregiver #1 reported completing one shelter in place drill during the year 2024.
