During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/20/2025, as a part of the licensure period. The on-site inspection began at 1:20pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 10 points. The inspector reviewed 5 children?s records and 3 caregivers records on-site. This inspection included: document review, tour of the facility, interviews, and observations.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
8have a current certification in CPR.
Assistant #2, whose CPR certificate expired over two years ago, was left alone with children for 10 minutes.
Part of the side fence was leaning and had exposed nails. There was a gap between the wood panels large enough for a child to fit through.
Poisonous cleaning chemicals were in an unlocked bathroom cabinet accessible to children.
There were no written parent authorizations on file for all diaper ointments.
Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
The record for child #1, #2, and #4 did not have a parent's signature for an annual review of the emergency contact information. Child #1's annual review was overdue by 4 months. Child #2 and #4's annual review were overdue by 3 months.
