During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on September 8, 2025. The on-site inspection began at 9:15am and ended at 11am. The inspector reviewed compliance in the areas listed above. There were 9 children present with 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
All employees shall undergo a background check and every five years thereafter. Of the 5 staff records reviewed, 2 of the 5 staff members have expired background checks.
Plan of Correction: The center responded with the following: The center will complete the background checks today and will ensure that all staff members have up to date background checks. Moving forward, all employees will have their background checks completed every five years.
Standard 22.1-289.035-B-4
Any individual required to undergo a background check shall authorize the child day center to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry in which the individual has resided in the preceding five years. Staff #2 disclosed on her sworn statement dated December 11, 2023, that she lived in another state outside of Virginia the past 5 years and no out-of-state background checks were completed.
Plan of Correction: The center responded with the following: The center will obtain the out-of-state background checks of Staff #2 and place the results in her record as well as send a copy to the Department of Education.
Standard 22.1-289.049-A
The child day program shall obtain the proof of the child's identity and age within 7 business days of attendance. Four of the five children?s records reviewed did not have proof of identity and age. All children were enrolled since last school year.
Plan of Correction: The center responded with the following: The center will reach out to the parents, and they will bring the child?s birth certificate for them to obtain a copy for the child?s record or either upload the birth certificate to the school?s online portal.
