Inspector Notes

An unannounced monitoring inspection was conducted on May 11, 2023. There were 6 children present (13 points) with 2 caregivers supervising. The home, children's records, caregiver records, household member records, attendance logs, evacuation logs cribs and first aid kits were reviewed. There were no medications on site. The children were observed playing with floor toys, coloring and listening to a story. Areas of non-compliance were cited on the Violation Notice. Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
-Child #1, #2 and #3's record do not contain documentation telephone number of each custodial parent's place of employment.
-Child #1's record does not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent can not be reached.
-Child #2's record does not contain documentation of the addresses of two designated persons to contact in case of an emergency if the parent can not be reached.
-Child #4's record does not contain documentation of the names of persons other than the custodial parents who are authorized to pick up child.
-Child #2 and #3's records do not contain documentation of the review of the child's emergency contact information as required by 8VAC20-800-780-B.
Plan of Correction: I will ask the parent's for the information and place information in each child's file.
