An unannounced, on-site monitoring inspection was initiated and completed on 5/30/2025. The on-site inspection began at 12:30pm and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 15. The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site on 5/30/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Child #1 was missing the Emergency Preparedness Plan, Proof of Liability insurance and the Medication Policy form.
Child #2 was missing the Emergency Preparedness Plan and the Proof of Liability Insurance.
Child #3 did not have an annual update of emergency information.
