Inspector Notes

An unannounced monitoring inspection was conducted on 06/11/2024 from 10:45am to 11:45am with the Provider. There were 3 non-household children ages 22 months to age 3 years old (14 points) with the Provider and an assistant. The programming area, attendance, outdoor play area, menus, Provider, Assistant and Household member records, 3 children?s records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in free play activities. The Provider had current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Based on documentation and Provider interview, the Provider did not practice monthly emergency evacuation drills with the children in care.

Evidence: The Provider provided documentation that there were no emergency evacuation drills completed in November 2023, December 2023, January 2024, and March 2024.

Based on review of 3 child records, the provider did not ensure they obtained documentation of additional immunizations for a child who is not exempt from the immunization requirements once every 6 months for children under the age of two years.
Evidence:
1)The record for Child #1 (age 2) did not contain immunizations once every 6 months for a child under the age of 2 years. The last immunization documentation contained in the record was dated 6/14/23 when the child was 18 months old.
2)The record for Child #2 (age 22 months) did not contain immunizations once every 6 months for a child under the age of 2 years. The last immunization documentation contained in the record was dated 8/11/23 when the child was 12 months old.
