Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 6, 2021 and concluded on April 7, 2021. The site administrator was contacted by telephone to initiate the inspection. There were eight children present and eight staff. The inspector emailed the site administrator a list of items required to complete the inspection. The Inspector reviewed two children?s records and one staff record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center failed to ensure that all required information was documented in each child's record.

Evidence:
1. The second emergency contact documented for child #2 did not have the complete street address for the contact person. The name, complete address, and phone number are required for two emergency contacts for each child.
Plan of Correction: This was corrected during the inspection. The site administrator called the parent and got the complete address. It was documented in the child's record.
