Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on September 11, 2023. There were 30 children ages 5 to 11 years old present under the direct care of 5 staff. The Licensing Inspector reviewed 5 children?s records and 6 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on inspection, the Center failed to post the fact that it is exempt from licensure in a visible location on the premises.

Based on inspection, the Center failed to ensure at least 1 person is trained to perform daily simple health screening & exclusion of sick children.

Based on record review, the Center failed to conduct a out-of-state sex offender registry check prior to the 1st day of employment for all prospective staff who have lived outside of the State of Virginia within the past 5 years.

EVIDENCE:
Licensing Inspector review 6 records. California Sex Offender Background Check for Staff #4 was not completed before employment; New York Sex Offender Background Check for Staff #5 was not completed before employment indicated. Both records did not document the date of employment. Both staff members were observed working during the inspection.
