Inspector Notes

An Initial Subsidy Inspection was conducted on August 5, 2024. A Religiously Exempt Code Inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in Religious Exempt requirements were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 16, 2024. A POC submitted after this date will not appear on the public website.

Based on record review, the Vendor failed to ensure all staff members are evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB).

EVIDENCE:
Staff #1 (date of hire 7/22/2012) did not have a tuberculosis skin test/screening.
