Inspector Notes
An unannounced renewal inspection was conducted 1/10/2024 between the hours of 10:55am through 12:45pm. There were a total of 9 children present, ranging in age from 15 months to 2 years (24 points) with the Provider and 2 assistants. A total of 5 children?s files, 3 staff files, and 2 household member?s files were reviewed. The children were observed engaging in self-directed play with a variety of age appropriate toys and materials and building with legos during the inspection. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. The provider advised there was no medication on site today. There was an abundant supply of age appropriate materials and toys and the home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

10Evidence: Staff #2 (Date of Hire: 6/5/2023) did not have Virginia Child Abuse and Neglect Central Registry search results on file.
Evidence: Child #2 (Start Date: 1/9/2023) and Child #5 (Start Date: 1/10/2022) did not have physical examinations on file.
Based on record review, the provider did not ensure that assistants' records contain all required information.
Evidence: Staff #3's record did not contain documentation or orientation training.
Evidence: Staff #1's most recent tuberculosis test results were dated 11/1/2021 and Staff #3's most recent tuberculosis test results were dated 9/10/2021.
Based on interview and documentation review, the provider did not maintain a written record of children in attendance each day.
Evidence: There was no attendance maintained for 1/8/24, 1/9/24 or 1/10/24.
Based on record review, the provider did not ensure that children's records contain all required information.
Evidence:
1. Child #2 (Start Date: 1/9/2023) and Child #3's (Start Date 10/22/2023) records were missing the medical insurance policy number.
2. Child #2 and Child #5's (Start Date: 1/10/2022) records were missing documentation of the provider verifying the proof of age and identity.
3. Child #3's record was missing immunization records.
4. Child #2 and Child #5's records were missing physical examinations.
Evidence:
1. Child #1 (Start Date: 1/17/2023), Child #2 (Start Date: 1/9/2023), and Child #3 (Start Date: 10/22/2023) did not have documentation on file that parents were provided the family day home's policies for the administration of medications.
2. Child #5 (Start Date: 1/10/2022) did not have documentation on file that parents were provided the family day homes provision of the emergency preparedness plan or documentation indicating if there is liability insurance in force on the family day home operation.
Based on observation of emergency supplies, the provider did not ensure that the first aid kit contains all required items.
Evidence: The first aid kit did not contain the required two triangular bandages.
Evidence: There was no documentation of shelter-in-place drills having been conducted.
Evidence: Child #3 (Start Date: 10/22/2023) did not have immunizations on file.
