During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/5/2025. The on-site inspection began at 10:30am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 27. The inspector reviewed 6 children?s records and 4 caregiver/household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/25/2025. A POC submitted after this date will not appear on the public website.

Child-resistant protective covers shall be installed on all unused electrical outlets accessible to children under five years of age.

There were 11 children present that are under age 5 and two outlets were observed without child-resistant protective covers.

High chairs shall be used according to manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.

There were four children ages 10 months, 10 months, 18 months, and 3 years sitting in high chairs eating lunch that did not have their safety straps fastened.

Child #6 has been enrolled for seven months and did not have the medical insurance policy number on file.
