Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including
documents review, and an in person of the
program (if applicable).

A monitoring inspection was initiated on Friday, February 4, 2022 and concluded on Tuesday, February 8, 2022 . The inspector conducted an in person inspection at the center to review compliance in the areas areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. There were 18 children present with 3 staff supervising. A total of 3
child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-4
Based on review and interview, the center did not ensure to obtain a copy of the results of a sex offender registry check, and a search of a child abuse and neglect registry or equivalent registry from any state in which the individual has resides in the preceding five years.

Evidence: In Staff#1's record (date of hire 11/22/21) there was no copies of required out of state background checks found for review. Interview with staff confirmed the out of state searches were unavailable for review during this inspection.
Plan of Correction: The director will make sure that all staff prior to hire have all the proper background checks when they have lived out of state.
Standard 8VAC20-780-500-A
Based on observation at the center, the center did not ensure all children's hands were washed with soap and running water after toileting/diapering procedures.

Evidence: In the center's Infant room, Staff#1 was observed completing a diaper change and Child#1 was placed on the carpet area with the other children without completion of handwashing procedures.
Plan of Correction: Staff 1 will get trained again on how to properly do hand washing procedures when it comes to changing a diaper.

Based on review of staff record and interview, the center did not ensure all required information was kept for each staff person.

Evidence: In Staff#1's record (date of hire 11/22/21) there was no documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position. Interview with staff confirmed there was no documentation available for review during this inspection.
Plan of Correction: The director will make sure that Staff 1 has all proper training courses for lead teacher role.
