A tuberculosis (TB) screening is required every two years from the last screening. Staff #4 updated screening was due 3 months ago. The updated screenings have not yet been completed.
Plan of Correction: I will make sue that all files are reviewed and assessed accordingly.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A container of disinfectant wipes with warning labels, ?Keep out of reach of children,? was unlocked in the cabinet under the sink in the hall bathroom, within reach of children.
Plan of Correction: All hazardous materials shall be locked and secured accordingly with the guidelines and rules.
